Broad Reach Hospice is seeking a qualified SOCIAL WORKER to join its team of dedicated healthcare professionals
PURPOSE OF POSITION:
In conjunction with the Interdisciplinary Team, the Social Worker participates in assessment, development of care plan, psychosocial treatment and discharge planning in both in-patient and home care settings.
ESSENTIAL FUNCTIONS:
l. Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of the plan of care.
- Conducts ongoing reassessments of patient/family needs and counseling as required.
- Provides short term crisis intervention and individual or family counseling when indicated.
- Observes, evaluates, and brings the team conferences information regarding psychosocial, spiritual, physical, and financial condition on affecting the patient and family.
- Assumes the active role of advocate for the patient/family unit.
- Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
- Documents comprehensive psychosocial information clearly and concisely in a timely manner.
- Participates in committees, task forces and agency events as required.
- Attends staff meetings, IDT and other meetings as assigned and appropriate.
- Participates in the orientation program as assigned.
- Participates in the development of and adheres to all Broad Reach Hospice policies and procedures.
- Assumes responsibility for own personal and professional development and maintenance of state-of-the-art skills in social work.
- Maintains current knowledge and practice of infectious disease protocols.
- Participates in clinical supervision as required by State law.

EXPERIENCE AND QUALIFICATIONS:
A Bachelor's or master’s degree in Social Work, Psychology or Counseling, and Massachusetts State Licensure at LSW, LCSW or LICSW level. Strong background in mental health/counseling and experience in health services delivery. Ability to work other members of the Interdisciplinary Team. Ability to conduct assessment of patient/family psychosocial history. Ability to identify and prioritize problems with appropriate goal formulation and conduct required interventions.
CONTINUING EDUCATION REQUIREMENTS:
Expected to participate in appropriate continuing education as may be requested and / or required by immediate supervisor and as necessary to maintain licensure. In addition, expected to accept personal responsibility for other educational activities to enhance job related skills and abilities, and to attend mandatory educational programs.
PERFORMANCE IMPROVEMENT AND COMPLIANCE PROGRAM REQUIREMENTS:
Expected to participate in Quality Assessment and Performance Improvement (QAPI) activities. Expected to comply with federal and state regulations governing hospice programs, and to adhere to Broad Reach Hospice's Code of Ethics as a condition of continued employment.
WORKING ENVIRONMENT:
Extensive travel to patient homes, hospitals, and nursing facilities throughout Broad Reach Hospice's service area. Travel in all weather conditions required. Potential exposure to blood, body fluids, and other infectious materials.
PHYSICAL DEMANDS:
While performing the duties of this job, the Social Worker is regularly required to use hands to finger, handle, or feel; reach with hands and arms; talk or hear; and taste or smell. The Social Worker is frequently required to stand, walk, and sit and to occasionally stoop, kneel, crouch, or crawl and lift and/or move up to 40 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Ability to drive required.
