RESPONSIBILITIES
- Provide homeless prevention case management services to households in the South Bronx.
- Determine eligibility at time of intake and every 90 days thereafter until case closure
- Assess level of housing crisis and create appropriate strategy to deal with the crisis
- Assess level of risk of client entering the shelter system
- Conduct initial home visit with new clients within 10 business days after case is assigned
- Complete service plan for each client and update at a minimum of every 90 days. Service plans should be accurate, detailed, and specific. All service plans must be completed with client input
- Complete all required funder's documentation
- Conduct Home Assessments, to ensure habitability standards are met
- Assess if financial assistance is appropriate and necessary to prevent housing instability.
- Complete a budget with each client
- Accurately complete and submit financial applications in a timely manner.
- Provide all necessary follow-up to meet agency and funder expectations to prevent shelter entry.
- Monitor and document client progress towards service plans
- Collect income documentation and proof of identification for all clients.
- Be well versed with the Housing Court process
- Understand all housing subsidies, offered within NYC
- Ensure that case records have detailed and thorough case notes
- Attend all agency and funder-required trainings.
- Perform additional duties as assigned.
QUALIFICATIONS
- Bachelor’s degree in social work or another related field of study.
- One-year of case management or relevant work experience preferred.
- Engaging personality and detailed orientated skills are essential.
- Strong oral and written communication, time management and organizational skills are necessary.
- Proficiency in Microsoft Office suite and other standard business technology is required.
PHYSICAL REQUIREMENTS
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds
- Ability to stand, walk, or sit for long periods of time
- Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
- Ability to bend and retrieve objects and/or documents
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
