BronxWorks is hiring (1) Program Specialist for the Targeted Prevention and Supportive Services (TPSS) Program.
RESPONSIBILITIES
- Provide direct services to clients, including intake, engagement, assessment, referral, service plan development, advocacy, counseling, exit summaries and follow-up.
- Monitor and document client progress toward service plan goals.
- Conduct home visits and other field work, as required
- Document all interactions with, or on behalf of clients
- Provide general clerical support, including filing, faxing, word processing, typing, photocopying, and sorting mail
- Answer telephones
- Report to, and meet with supervisor monthly basis or more frequently as required
- Assist with special projects as assigned
- Perform additional duties as assigned
PROGRAM SPECIFIC RESPONSIBILITIES
- Provide direct services and prevention education clients living with HIV/AIDS (PLWH/A) and people who are at high risk for contracting HIV, sexually transmitted illnesses (STI's) and Hepatitis C, including people engaging in high risk sexual behavior and intravenous drug users.
- Drive agency medical van to specified testing and outreach locations for the purpose of conducting testing and outreach responsibilities
- Fill vehicle with gas when needed.
- Notify supervisor of vehicle maintenance problems as they arise.
- Manage a caseload
- Administer HIV and HCV Testing
- Facilitate evidence-based interventions, including, Safer Sex in the City, Healthy Relationships and VOICESNOCES.
- Maintain familiarity with program resources available on- and off-site
- Maintain company property such as agency cell phone and/or computer.
- Ensure that Verizon FFM is active and report to supervisor if app is not working.
- Act as a program contact person for inquiries concerning clients
- Coordinate client services with other social services, mental health, medical and other providers.
- Refer and link clients to necessary medical and supportive services.
- Complete intake, risk assessment, service plan development, referrals, advocacy, and counseling.
- Conduct Outreach.
- Perform additional duties as assigned.
QUALIFICATIONS
- Prefer at least one year of experience working with people living with HIV/AIDS. Prefer one year of experience doing HIV rapid testing and Hepatitis C rapid testing, although training will be provided.
- Minimum High School Diploma, Associate Degree preferred.
- Proficiency in English required; Proficiency in second language preferred.
- Proficiency in modem business communications, including personal computers, electronic mail, voicemail, and facsimile and copier equipment.
- Valid NYS Driver's License preferred.
PHYSICAL REQUIREMENTS
- Full vaccination against COVID-19 is required.
- Good communication, writing interpersonal and organizational skills
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
