FULL TIME- MONDAY - FRIDAY - DAYS - 80 HOURS PER PAY PERIOD
Licensed Counselor Social Worker (LCSW) in the State of Florida with experience in Home Health and/or Hospice preferred. Ability to perform assessments/visits, counsel and screen Home Health clients in the hospital and home settings in compliance with regulatory Medicare/Medicaid guidelines. Ability to access available financial and social resources for patients/families in order to meet State and Federal guidelines.
Master's degree in social work obtained through at least a 2-year graduate program. Current Florida Social Work License. American Heart Association BLS. At least one year of related experience.
