General responsibilities:
1. Provide hands on nursing care under the direction of the RN at admission and throughout the hospitalization with a focus on cultural, spiritual, physiological, psychological, and age related needs.
2. Perform all treatments and procedures in accordance with established policies and procedures.
3. Participate in the admission process by gathering information related to the physical and psychological needs of the patient and reports this to the RN responsible for the patient.
4. Identifies and prioritizes current and potential problems in collaboration with the RN.
5. Accurately document pertinent patient information in accordance with policies and procedures.
6. Prepares patient and family for discharge and follow up.
7. Provides the patient with the following medication services: administration, monitoring pre and post evaluation of the patient’s response to the medication administration within the scope of practice.
8. Brings any issues to the attention of the RN responsible for the patient.
9. Gives / receives appropriate information at shift change and throughout the shift using clear and complete communication following the SBAR formula.
10. Maintains unit equipment as directed by the nurse manager and supervisors.
11. Practices standard precautions
12. Performs other work-related duties as assigned
Education and Experience Requirements:
Please note: All employees are expected to adhere to NYS mandate and be fully vaccinated against COVID prior to start date. Currently offering a $2000 sign on bonus for LPN positions.
