About the Opportunity
Byte Back is currently recruiting an outstanding case manager to support our incredible program participants in Baltimore. This position will provide coordination for Scholars and provide meaningful support through coordination of services, outreach, and sourcing additional resources for social, health, and medical needs. This opportunity requires a great deal of hands on, on call, care to ensure program participants achieve goals, reduce unmet needs and concerns, and are receiving all necessary support from wrap-around services. The position offers the opportunity to be primarily home-based with the ability to travel to our Baltimore office with a hybrid work schedule.
About Your Responsibilities
Your responsibilities will include, but are not limited to the following:
- Establish a positive rapport with program participants while maintaining strict professional boundaries
- Relentlessly show up and be present in the lives of our Scholars, especially through times of crisis
- Assists participants in a manner that demonstrates respect, honesty, non-judgment and empathy
- Complete intake for all new Scholars entering Byte Back programs including an intake assessment, initial case plan, releases of information, risk assessments, and safety plans as needed;
- Develop and facilitate group life skills workshops
- Manage a caseload of 30-40 adult learners
- Helps develop and write, monitor and update Scholar's Individual Case Plan in collaboration with the Scholar, and with external service providers
- Monitor and track progress of all program participants by making the necessary referrals for critical interventions, both internal and external
- Present cases during "Byte Back Scholar Journey" update meetings
- Meet with program participants weekly to address and monitor progress
- Provides case management support to dedicated cohorts which includes but not limited to identifying and addressing psychosocial concerns, engagement in services, coordinating care, advocacy, and health related, financial, and housing barriers
- Provides assistance in scheduling appointments and accessing community support services as needed.
- Participate in weekly case reviews and transition meetings
- Provide crisis management support to any Scholar identified as "in crisis"
- Ensure timely and accurate documentation of assessments (e.g. Intake, Initial Case Plan), progress notes, case plans, verbal encounters, document needing to be uploaded
- Reports programmatic needs or concerns to the Site Director and/or Head of Programs
- Commit to engage in personal and professional growth and competence development, participate in organization training activities to increase capacity to serve Scholars every day
- Attends trainings and meetings as required.
- May be required to work special events and perform other duties as assigned
About Your KSA's
- Possess a minimum of 4 years of demonstrated accomplishments in a case management role
- Bachelor's degree in Human Services, Social Work, Social Science, or Psychology or equivalent preferred but not required
- A demonstrated commitment to high professional ethical standards and ability to exercise discretion and independent judgment
- Excellent oral, written, grammar, proofreading and communications skills that demonstrate capacity to present a compelling case, persuade, engage, and excite while incorporating creativity and ensuring accuracy
- Must be very approachable, build exceptional rapport, and practice attentive and active listening with a strong attention to detail and organization skills
- Demonstrated ability to utilize trauma-informed care concepts and a strengths-based approach to service delivery especially when addressing individuals in crisis
- Strong and demonstrable understanding of adult development and behaviors; demonstrate enjoyment of and ability to relate to our Scholars and program participants
- Extensive knowledge of Baltimore-based community resources; experience working directly with Baltimore-based orgs and programs that provide diverse supports and social services to community members
- Demonstrates cultural sensitivity and can work and relate to a diverse population and demonstrate cultural competence with historically excluded communities
- Excellent interpersonal skills, and high comfort level working in diverse environments
- Computer savvy and high proficiency with a spectrum of technological products and web-based software including Google Suite tools, Salesforce, and Zoom; experience with case management software platforms preferred
- Ability to work independently and with limited supervision in a virtual work environment
- Excellent time management and prioritization skills
- Possess a proven ability to work collaboratively as part of a team and an interest in building a strong organizational culture
- Commitment to maintaining the highest levels of confidentiality
