Roles and Responsibilities:
- Organizing and delivering client-centered care that is safe, efficient, productive, economic, and equal1
- Conducting intake assessments with patients and families, and liaising with physicians and specialists on accurate diagnosis
- Preparing, implementing and monitoring individualized treatment plans that account for physical, financial, and other constraints
- Managing patients to the appropriate level of care and ensuring optimum utilization of resources3
- Providing referrals for external mental and physical health services, and assisting customers in achieving wellness and self-sufficiency
- Tracking patient progress and adjusting treatment protocols, as well as deciding on case closure
Job Types: Full-time, Contract, Travel nursing
Pay: $2,900.00 per week
