LPN Home Health Intake Coordinator
Responsibilities:
- Process all referrals for Medicare certified home health services
- Qualify all intake requests for both clinical appropriateness and financial reimbursement
- Ensure all intake data is accurate
- Articulate an understanding of Skilled Home Health services in all communication with referral sources and team members
- Exhibit excellent customer service
- Work closely with scheduling, Billing, and Quality departments
Qualifications:
- Licensed Practical Nurse
- Minimum 1 year clinical experience
- Proficient computer skills with data entry
- Home Health experience preferred not required
- Excellent customer service skills
