JOB SUMMARY

The Health Information Clerk manages health information data by ensuring its quality, accuracy, accessibility and security in both paper and electronic systems. Performs a variety of technical and clerical functions in accordance with departmental policies and procedures to maintain records and collect data. Ensures that records are completed timely and accurately. Responsible for assuring records are routed to the coder analysts and appropriate healthcare provider for timely and accurate completion. Ensures accurate and timely statistical reporting for the HIM department and organizationally. Ensures the medical record integrity through scanning, filing, copying, assembling, and retrieving patient records. Releases protected health information according to federal, state and departmental policies. Notarizes documents as necessary. Performs other duties as assigned.

ESSENTIAL FUNCTIONS

  • Utilizing AIDET, answers incoming phone calls and greets visitors to the department promptly.
  • Utilizing AIDET responds promptly and courteously to walk-in requests from patients for copies of health care information.
  • Receives and sorts and distributes department mail.
  • Retrieves records from appropriate clinical areas.
  • Provides general clerical support to include distribution of materials, filing, faxing and copying. Assists with filing loose reports on records.
  • Organizes and maintains data for the EMR and registries.
  • Identifies duplicate medical records.
  • Updates incomplete records timely and efficiently.
  • Compiles physician suspension list for incomplete or delinquent medical records according to departmental standards.
  • Processes birth verifications following departmental procedure.
  • Analyzes and processes records according to departmental procedures and maintains department standards.
  • Releases confidential information according to departmental policies.
  • Responds verbally or in writing to requesters, as appropriate, regarding requests which cannot be processed due to lack of information, misinformation, missing or inadequate release of information form, and delays due to incomplete medical records.
  • Scans medical records into the archiving system according to departmental policy.
  • Must be free from governmental sanctions involving health care and/or financial practices

  • Complies with the hospital’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.

JOB QUALIFICATIONS

  • Education
    • High school diploma or GED
    • Medical terminology preferred.
  • Licensure
    • None
  • Experience
    • Previous experience in healthcare field preferred.
    • Experience in a hospital-based medical records department preferred.

Knowledge, Skills, and Abilities:

  • Computer Knowledge including the ability to enter material into electronic record. Ability to operate fax, computer, and copy machine to perform the duties of the position.
  • Excellent interaction with visitors, patients, coworkers, medical staff, outside agencies effectively and courteously. Corresponds effectively with outside agencies.
  • Must possess attention to detail. Review of requests for appropriate criteria. Ability to interpret and apply federal and state statutes regarding the disclosure of healthcare information.
  • Outstanding administrative and organizational skills, professional telephone skills and a strong customer service mindset and style.
  • Ability to edit, proofread and compose written materials, to organize and prioritize workload, and to work with computers and various software programs. Requires completion of tasks under deadline. Ability to think quickly and handle various projects simultaneously.
  • Analytical and reasoning skills with a high level of mental concentration and memory.
  • Ability to organize priorities and meet specific objective under time constraints.
  • Ability to manage multiple tasks simultaneously.
  • Adheres to customer service standards and promotes a service orientation in the performance of job duties and responsibilities and interactions with patients, visitors, and other staff members.
  • The ability to maintain continuing education requirements and all certifications and licenses as required (e.g. ACLS, PALS, CPI, CPR).
