Cancer Support Community is an international non-profit organization providing professionally led support groups, educational workshops, and healthy lifestyle classes for people facing cancer and their families. There are four affiliates in Southern California: CSC Greater San Gabriel Valley, CSC Los Angeles, CSC SouthBay, and CSC Valley Ventura/Westlake Village (collectively, CSC SoCal).
Position Title: Psychosocial Navigator
Reports to: Program and Clinical Director
Position Type: Full Time (40 hours per week), Exempt
Position Overview: The Psychosocial Navigator’s position is twofold:
1) 20 hours/week: For CSC Greater San Gabriel Valley, as part of the Program staff, to perform duties and responsibilities set forth below; and
2) 20 hours/week: For the CSC SoCal initiative, to serve as the liaison between CSC SoCal and City of Hope patients interested in participating in the programs and services offered via CSC SoCal.
CSC Greater San Gabriel Valley Essential Duties and Responsibilities:
· : Utilize new participants’ distress screening results to inform and provide case management services and resource connection. Manage drop-in or phone inquiries related to navigation and provide emotional support to participants when needed. Identify and engage local service providers to be included on resource list. Complete documentation of navigation services.
· : Assist with new participant onboarding procedures by processing orientation attendance and intakes and following up with participant communication. Maintain support group enrollment and attendance tracking.
· : Plan, coordinate and execute educational workshops, healthy lifestyle classes, social activities, and provide resources/referrals; Program promotion, program set up, communication with instructors/speakers, and program evaluation. Overall administrative support in Program department, including data entry, preparing correspondence, following-up with member questions and staff needs.
CSC SoCal Essential Duties and Responsibilities*:
As part of the Cityof Hope partnership initiative, CSC SoCal will design a virtual weekly orientation specifically tailored to support a major medical institution. This orientation will serve as a gateway for patients to access a wide range of psychosocial services available across the Southern California region.
Through this virtual orientation, patients of a major medical institution will no longer need to designate a specific Cancer Support Community location to receive services. Instead, they will have immediate access to a diverse array of programs and services without geographical barriers. This approach significantly enhances the options available to patients of major medical institutions, providing them with access to more than 68 weekly support groups and 43 weekly healthy lifestyle classes. This is a substantial increase from the average of 12 offerings typically available at each location.
Additionally, this partnership will extend to COH patients other services such as short-term counseling, child, teen, and family programs, educational workshops, social events, and more. Through this collaboration, CSC SoCal aims to provide COH patients comprehensive and seamless access to the support and resources they need during their cancer journey.
Orientation Coordination:
· Coordinate the scheduling and logistics of the virtual weekly orientation.
· Ensure that the orientation provides comprehensive information about the psychosocial services available across the SoCal region.
· Administer Support Care Plan (Distress Screening) and Intake Interviews; Perform new member follow-up calls (Spanish and English).
· Provide intake interviews and customized support plans as needed and in coordination with the CSC SoCal Clinical and Program Directors.
Resource Coordination:
· Work with internal teams to compile a comprehensive list of psychosocial services available across the Southern California region.
· Provide patients with information about the various services available to them, including support groups, healthy lifestyle classes, counseling services, and more.
· Assist with Spanish-Speaking membership recruitment efforts in order to extend reach of services to a broader population.
Program Promotion:
· Promote the virtual orientation and other psychosocial services to COH patients to increase awareness and participation.
· Ensure that patients understand they no longer need to designate a specific Cancer Support Community location to receive services, eliminating geographical barriers.
Documentation and Reporting:
· Maintain accurate records of patient outreach, orientation attendance, and services provided.
· Provide regular reports to management on the effectiveness of the referral system and virtual orientation.
· Provide documentation for all patient contact, including intake and progress notes, support group attendance, contact logs, authorizations to release information, etc.
Collaboration and Communication:
· Collaborate with COH medical teams and internal teams at CSC SoCal to ensure effective communication and coordination of services.
· Participate in regular meetings and discussions to provide updates on the partnership and address any issues that arise.
· Identify and build relationships with relevant community organizations and hospital systems to build database of local resources and steward robust partnerships.
· Act as an agency representative at outreach events and satellite partnerships.
Continuous Improvement:
· Continuously evaluate the effectiveness of the referral system and virtual orientation.
· Identify areas for improvement and work with internal teams to implement changes as needed.
· Attend regular training group supervision meetings, clinical team meetings, staff meetings, admin meetings, and individual/triadic supervision sessions as appropriate to responsibilities and pre-licensure requirements.
Adherence to Policies and Procedures:
· Adhere to all policies and procedures related to patient confidentiality, data protection, and service delivery.
· Ensure compliance with all regulatory requirements related to the provision of psychosocial services.
· Lead new participant orientations to welcome new Spanish-Speaking participants and assist participants with registration as needed. Work to continuously review and improve the onboarding process.

Education:
· Bachelor’s Degree in Social Science (Psychology, Social Work, Human Development and Family Studies, etc.)
· Experience: Previous experience in counseling or social work preferred.
Qualifications:
· Bilingual in Spanish and English
· Knowledge of cancer support services and resources
· Strong writing, communication and interpersonal skills
· Ability to prioritize and organize activities while working within deadlines.
· Self-motivated with initiative and independent actions within the scope of responsibilities.
· Working knowledge of Microsoft Office Suite (Word, Excel, PowerPoint), Google Docs, Dropbox).
· Ability to work flexible hours, including evenings and weekends.
· Employee must have access to transportation to travel between each of the two pilot program site locations 1x/week required
Characteristics:
Evaluation: The Psychosocial Navigator's performance will be evaluated based on the successful completion of assigned duties and responsibilities.
Work Environment: This is a full-time position, with a 4-day/10 hour work week (Monday – Thursday. Office is located in Sierra Madre, CA; Pilot program sites in South Pasadena and Upland. Must be able to work a flexible schedule, including some nights and/or weekend work and some possible travel. Valid Driver’s License required. Reliable transportation and insurance necessary. Able and willing to lift/move event related materials weighing up to 25 lbs.
