Candid Home Health Care, Inc is looking to hire EXPERIACED Registered Nurses and LVN
for patient home visits.

Job Requirements:

We would prefer if the candidates will have at least one year of experience with home health agencies and have experience with patient home visits and maintaining their medical records

The RN position requires experience and knowledge with opening SOC cases, creating OASIS for Medicare, assign visit frequency for the clinicians treating the patient during their treatment episode ( PT , LVN , OT , CHHA .. etc..) , knowledge of Synergy software is a huge plus.

LVN position requires experience and knowledge of conducting patient home visits and following the treatment plan , maintaining their visit notes , ability in maintaining the visit notes in Synergy software is a plus.

The schedule can be full time, part time , per deem.
