The Telehealth Registered Nurse (RN) role offers an exciting opportunity to work in a cutting-edge environment, shaping the future of healthcare by providing telehealth nursing care to patients at home or in a non-office setting to achieve optimal health outcomes. Specifically, the Telehealth RN will service a book of patients and provide virtual health and wellness coaching based on the interpretation of health vitals. This is an excellent opportunity to work with a dynamic team of Clinicians that will include Doctors, Nurse Practitioners, Physician Assistants, and Registered Nurses who are dedicated to enhancing remote patient care.
JOB FUNCTIONS
- Meets with patients via audio or video calls to discuss condition management and to review prior month's health vitals
- Monitors patient alerts
- triage, review, respond, and document as appropriate
- Reviews trends in patient data and changes in condition and documents notes in patient account
- Documents interventions and an individualized plan of care
- Educates patient on the meaning of their health vitals
- Educates patient on preventative care options which promotes health and well being
- Utilizes a standardized treatment approach to provide high level of care to patients
- Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery
- Serves as advocate for patients and families in response to their needs
