GENERAL DESCRIPTION SUMMARY
The Nights & Weekends RN is to provide visits to patients with emergent, critical needs in a timely manner. The Nights & Weekends RN arranges for, teaches, and supervises nursing care of patients in home care and facility settings in collaboration with other members of the Interdisciplinary Group. The Nights & Weekends RN makes nursing assessments and uses critical thinking skills to provide care based on evidence-based practice.
Essential Duties & Responsibilities - Within the philosophy, objectives and policies of Capital Caring Health, carries out the following essential functions:
Quality Provision of Services:
Delivers or supervises the delivery of skilled nursing services as outlined in the plan of care with the focus on palliative & comfort care according to Capital Caring Health policies and procedures and within the guidelines of the appropriate Nurse Practice Act. Collaborates with Patient Care Advice Line staff, After Hours Coordinator, Clinical Supervisor, Clinical Manager, Hospice Medical Director, Attending Medical Doctor, Crisis Care staff, and patient/family to determine and implement patient care needs and revises the plan of care as needed. Teaches and supervises the teaching of caregivers responsible for the daily personal care needs to empower the caregivers to meet patient needs. Initiates appropriate referrals to the other disciplines and/or other community resources to assure patient needs are met.
Organizational/Regulatory Compliance:
Completes all documentation of nursing services delivered, communication, and physician orders the same day using appropriate forms/formats according to Capital Caring Health policies and procedures. Assumes responsibility for own professional growth in order to improve, maintain, and develop new skills by participating in ongoing education and updating knowledge of current nursing practice through journals, literature review and obtaining needed continuing education hours for licensure maintenance. Bases skills on evidence-based practice. Evaluates own needs for support and uses identified system(s) to meet the need. Participates in quality improvement activities, QUAPI program and hospice sponsored in-service training. Serves on committees or teams as approved by the clinical supervisor.
People/Communications:
Communicates with Patient Care Advice Line staff via Capital Caring Health email/text pager for delivery of needed information related to patient contact requests. Communicates with the Attending Medical Doctor, Hospice Medical Director and the remainder of the Interdisciplinary Group to report changes in the patient’s medical and mental condition. In collaboration with the Clinical Supervisor, may precept new Capital Caring Health employees assigned to the nursing service. Functions as a member of the team by demonstrating flexibility in work schedules and shared responsibility for delivery of care to assure that all regions are well-staffed in the after-hours. Per agency policy, is available to work a minimum of one major & one minor holiday per the calendar year.
Financial Stewardship
Achieves expected productivity standard as defined by the organization and in alignment with best practices in Capital Caring Health’s Hospice Model. Uses good stewardship in the provision of supplies and services as needed for implementation of care. Initiates the abbreviated admission process when required for patients in acute need of services.Conducts informational/liaison visits as requested when the Nights & Weekends schedule permits.
