Responsibilities
- Processes physician orders that includes critically evaluating drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose documented by intervention reports.
- Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection by processing non-formulary drug requests and documentation.
- Provides clinical consultation and clarification to practitioners and patients.
- Supervises and directs pharmacy support personnel
Requirements
- Bachelor's degree in Pharmacy. Graduate of an ACPE accredited school of pharmacy or equivalency.
- One year of hospital related pharmacy experience preferred.
- BLS.
- Valid NJ License.
- Frequent physical demands include: Sitting , Standing , Walking , Pinching/fine motor activities Occasional physical demands include: Climbing (e.g., stairs or ladders) , Carry objects , Push/Pull , Twisting , Bending , Reaching forward , Reaching overhead , Squat/kneel/crawl , Wrist position deviation Continuous physical demands include: Keyboard use/repetitive motion , Talk or Hear Lifting Floor to Waist 15 lbs. Lifting Waist Level and Above 10 lbs.
