Job Description:
Capitol View Transitional Care Center has an opportunity for a Social Worker to join our team. Capitol View Transitional Care Center is a 32-bed transitional care center that specializes in intensive, short-term rehabilitation providing inpatient therapy and nursing care for patients after hospitalization. Capitol View is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital (we are part of the HealthPartners organization as a separate nonprofit organization.)
The Social Worker provides medical social services and interventions to our patients including:
- psychosocial and mood assessments that includes initial chart review and assessments, completion of ongoing assessments of patient mood and documenting in electronic medical record including care planning and other pertinent case information
- providing ongoing care, discharge and transition planning and coordination
- providing consultation and collaboration with outside organizations and professionals regarding the case
- communicating with patients about planning and acting as a facility contact for family members
- offering direct services and interventions to patients and families and seeks to promote the maintenance of patient health, especially mental health
- providing supportive counseling to patient and family members regarding psychosocial issues related to patient stay
- assisting patient and family with understanding of insurance coverage and limitations
- coordinate and facilitate a care conference as needed
Required Qualifications:
Bachelor’s degree in Social Work
One (1) year of social work experience
Licensed as LSW or higher
