Carbon County Area Agency on Aging has an open Aging Care Management Supervisor II position. This local government position is responsible for supervising the Protective Services program staff and their caseloads of vulnerable clients.

OVERALL OBJECTIVE OF JOB:

To oversee, supervise and direct the Protective Services Program, assigned staff and

their assigned caseloads to ensure programs are being carried out in accordance with

established policies, procedures and guidelines and that clients are receiving the best

quality care and programs.

ESSENTIAL FUNCTIONS OF JOB:

1. Directly responsible for the Protective Services program and serves as the ‘back

up’ supervisor for all other Agency programs.

2. Ensures Protective Service cases meet the guidelines and expectations of

Pennsylvania Department of Aging.

3. Ensures all Protective Service cases adhere to Protective Services documentation

guidelines.

4. Ensures all Protective Service cases are completed within the timelines set by

Pennsylvania Department of Aging.

5. Reviews and submits for approval for all PS expenses, and ensures that adequate

documentation is in the file.

6. Prepares the Annual Protective Service plan as outlined by the Pennsylvania

Department of Aging and submits to Administrator for final review and submission.

7. Ensures that all PS staff, primary and back up receive annual enrichment training.

8. Provides assistance to Protective Services care managers for caseload

management, which would include providing emergency back-up coverage in

absence of the assigned Protective Service worker.

9. Develop and conduct Protective Service presentations designed to develop

performance and case management skills to other organizations/audiences with

the current rules, regulations and procedures in the field of social services.

10. Recommends and prepares Protective Service cases for court action; involuntary

action and/or Guardianship.

11. Makes referrals to care managers/case aides and other assignments as required.

12. Reviews, approves all services assigned and assessments for each client.

13. Ensures timely documentation of all supervisory meetings with staff.

14. Acts as liaison between service providers and Agency in regards to all Aging

services and other related tasks.

15. Interacts with staff, other agencies, and government offices as necessary to carry

out essential functions of job.

16. Assists in training of staff; supervises staff and handles related personnel matters,

including performance evaluations, work assignments and employee time sheets.

17. Maintains files, records and documentation; completes reports as required by

Pennsylvania Department of Aging.

18. Assists in maintaining waiting list for services.

19. Maintains confidentiality of case details at all times.

20. Assists in the developing and implementing of Agency policies and procedures.

21. Assists in planning budgets as needed.

22. Collaborates with Administrator and other supervisors in the exchange of information, workflow, and communication of ideals to improve agency operations and service delivery.

OTHER DUTIES OF JOB:

1. Attends training, meetings, seminars as required both in and out of county.

2. Prepare any Annual plans as requested.

3. Performs other job-related duties as required.

SUPERVISION RECEIVED:

Receives occasional instruction and some supervision from Administrator in regard to daily work duties.

SUPERVISION GIVEN:

Supervises 3-5 assigned staff members.

WORKING CONDITIONS:

1. Works indoors in adequate work space, temperatures, ventilation and lighting.

2. Frequent exposure to noise, disruptions and stress.

3. Normal indoor exposure to dust/dirt.

4. Periodically works on as-needed basis.

5. Travels as necessary and required to perform essential functions of job.

6. Potential exposure to blood borne pathogens.

PHYSICAL/MENTAL CONDITIONS:

1. Must possess ability to record, convey and present information, explain procedures and follow instructions.

2. Must be able to sit for long periods throughout the workday, with intermittent periods of standing, walking, bending, twisting, and reaching as necessary to carry out job duties.

3. Dexterity requirements range from simple to coordinated movements of fingers/hands; feet/legs; torso as necessary to carry out duties of job.

4. Must be able to cope with the physical and mental stress of the position.

5. Must be able to pay close attention to detail and concentrate on work.

QUALIFICATIONS:

A. EDUCATION/TRAINING:

Bachelor's degree in social work, psychology, sociology or human service-related field; computer training, first aid and CPR certification also preferred.

B. WORK EXPERIENCE:

4 years’ work experience in public or private social work. Or, any combination of acceptable training and/or experience which includes 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social services.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

1. Must be able to speak and understand the English language in an understandable manner in order to carry out essential functions of job.

2. Must possess good communication and interpersonal skills.

3. Must possess initiative and problem-solving skills.

4. Must possess ability to function independently, have flexibility and the ability to work effectively with clients, co-workers and others.

5. Must possess ability to maintain confidentiality in regard to client information and records.

6. Represents Carbon County AAA and the County of Carbon in professional manner at all times.

7. Must possess a willingness to travel as needed to carry out essential job duties.

8. Must possess the ability to make independent decisions when circumstances warrant such action.

9. Must possess knowledge of the principles and practices used in social service work, and the ability to apply these principles and practices within the job.

10. Must possess some knowledge of the medical concerns and related treatments and practices regarding the aging process.

11. Must possess some knowledge and ability to counsel elderly and to work in crisis situations as necessary.

12. Must possess knowledge of community resources and the ability to interact effectively with them on clients' behalf.

13. Must possess ability to conduct complete assessments and to determine most appropriate, least restrictive level of care needed.

14. Must possess knowledge of and ability to practice principles and methods of effective management and supervisory skills.
