What are the responsibilities and job description for the Case Manager/Community Support Specialist position at Cardinal Cushing Centers?
At the Cardinal Cushing Centers, we support people with intellectual and developmental disabilities, providing possibilities and opportunities for growth and advancement. Our programs for students, adults and community members include education, employment training, transition services, residential care, therapeutic and recreational services.
Provide supports to the AFC members and families. Assesses adult medical issues, administers medication as needed, and handles specific responsibilities assigned. Complete administrative assignments for designated caseload.
. The responsibilities of the care manager include:
1. Conducting the initial and ongoing assessments of the AFC qualified setting;
2. May be asked to take on the role of a Community Support Specialist, as needed; which requires developing an individualized training for that one visit.
3. Obtaining a social history and conducting a psychosocial assessment and evaluation;
4. Assist with the selecting, training, evaluating, and supervising of the AFC caregivers in conjunction with the registered nurse;
5. Participating in the development, implementation, and ongoing review of the AFC plan of care;
6. Assist with the implementation of any CARF and MassHealth regulations
7. Conducting on-site and or telehealth visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the registered nurse) for Level I, and monthly for Level II according to current AFC regulations;
8. Completing a care-management progress note corresponding with each on-site visit or encounter and upon significant change;
9. Assisting with obtaining information and accessing other health-care, mental health, community and DDS services.
10. Reviewing and documenting the fire drills and safety procedures for the qualified setting;
11. Participating in discharge planning and implementation
12. Participate and/or conduct AFC audits, year-end closers and book reviews
- Providing training to AFC caregivers on health and aging as needed
- Willingness to use your personal automobile to get to and from home and program sites.

Related Work Experience
Minimum of five years experience working with people with disabilities.
Formal Education, Certification, Licensing
A bachelor’s degree, a social work license from the Massachusetts Board of Registration in Social Work, and at least two years recent experience working with elderly or adults with disabilities; or
A bachelor’s degree and five years’ clinical experience in the care of elderly or persons with a disability.
(a masters degree can be substituted for two of the five years of clinical experience)
Valid driver’s license required.
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
