Summary:
As a member of the palliative care team, the social worker reports to the Clinical Director of Palliative Care, and is responsible for planning and providing supportive services to patients with life-limiting, complex illness and their families. The Palliative Care Social Worker (PCSW) will work collaboratively with other professionals in the organization, other health entities and in the community to coordinate and provide an additional layer of support for patients with advanced illness.
The social worker is responsible for initiating discussions on the social and emotional factors related to the patient’s illness; facilitating discussions to clarify goals of care; and for identifying patient/family psychosocial needs. Addresses bridge to hospice care when indicated. The Palliative Care Social Worker interprets for the patient, family and members of the care team, factors related to illness, financial and community resources.
What You'll Bring:
- Master's degree in social work or related field required.
- Current Massachusetts social work licensure (LICSW preferred, LCSW accepted with appropriate work experience).
- Minimum three years counseling experience preferred.
- Experience counseling persons with life-limiting, complex illness and their families; knowledge of the health care delivery system and community resources.
- Experience with hospice, community based support services and/or home care beneficial. Experience with palliative care preferred.
- Demonstrated ability to organize, prioritize and complete multiple tasks in a fast-paced environment.
- Demonstrated ability to communicate professionally and effectively in person, via telephone and via video conference.
- Some experience in leading support groups preferred.
- Demonstrated comfort with and ability to use appropriate technology effectively.
- Demonstrated empathy towards and ability to interact appropriately with patients of diverse social backgrounds, economic status, diverse cultures and individual lifestyles.
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
- Demonstrated understanding of palliative care and hospice philosophy and principles.
- Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation, and in ability to work effectively within an interdisciplinary team.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.
