HOME HEALTH INTAKE COORDINATOR

JOB DESCRIPTION

The Intake Coordinator is responsible for effectively managing daily office operations and scheduling clinical staff to meet the needs of the patients according to company policy and in accordance with Federal and State regulation. The coordinator will perform various clerical activities and related tasks including but not limited to answering incoming calls, scheduling appointments, the flow of correspondence, filing, taking copayments/deductibles, and Office Assistant functions.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES

1. Coordinates all aspects of patient care including intake, chart creation, scheduling/staffing visits, chart audits, orders, etc.
2. Acts as a liaison between patients, clinicians, and administration.
3. Communicates effectively with patients and their families, clinicians, and all staff.
4. Prepares all meeting agendas & maintains accurate up to date clinical records
5. Performs other necessary functions/duties as assigned by the Program Director and/or Administrator

QUALIFICATIONS

1.Minimum 0-1 years experience in home health care intake and/or scheduling.
2. Willing to Train
3. Must be detail-oriented and thorough follow-up skills.
4. Strong verbal and written communications skills.
5. Strong computer skills for data entry, other programs.
6. Ability to coordinate high volume of patient and therapist appointment schedules.
7. Ability to multitask in a fast paced, high energy environment.
8. Troubleshoot, prioritize and present a polished, positive image to referral sources, co-workers, field staff, patients and families.
9. Provide occasional translation to caregivers and staff
