Case Manager Transition Services

Job Summary:
You will provide direct case management services to adults with a primary mental health disorder who reside in Bergen County and are at-risk or involved in the criminal justice system in order to divert incarceration and reduce recidivism. You will spend 65% of your time face-to-face with consumers, in their homes, and in the community.

What You Will Do:

• Assess and evaluate consumers at intake, admission, during ongoing treatment, service delivery, and at discharge or release from incarceration.
• Collaborate with treatment providers to establish comprehensive assessments and service plans to increase tenure outside of the correctional system.
• Conduct outreaches, assess consumers, and communicate the integration of consumer history and information for evaluation
• Create psychological service plans containing relevant, behaviorally measurable goals and realistic time frames for achievement.
• Provide substance abuse services, family consultation, crisis intervention, vocational, educational, employment services, and comprehensive services based on the Risk of Hospitalization.
• Assist consumers in completing appropriate financial and insurance forms
• Monitor team functioning to ensure consumers receive the required care and research and coordinate resources for service provision and training.

What We Need to See

• A Master's degree OR Bachelor's degree in Psychology or a related field, plus at least one year of experience working with individuals with severe and persistent mental illness.
• A valid driver's license must be insurable by the agency's automobile insurance.
• Case management experience is preferred.
