QUALIFICATIONS:
An associate degree and three years’ experience in the social service or criminal justice field. Education may be substituted with relevant experience on a one year for one-year basis.
About Project Longevity: Project Longevity is an initiative intended to be a multi-partner community collaboration and engagement activity between the police departments, community advocates and non-profit social service organizations to reduce gun violence in the Bridgeport, Hartford, Waterbury, New Haven, Norwich, and New London areas.
GENERAL FUNCTION:
In conjunction with staff of the Bridgeport Reentry Welcome Center, the Care Coordinator will receive referrals from Project Longevity staff and assess clients’ needs in order to make appropriate referrals to community based social service providers. The Intensive Case Manager will work in tandem with another Intensive Case Manager and be working with a population of at-risk men and women in the City of Bridgeport.
JOB RESPONSIBILITIES:
· Case Management of Project Longevity clients
· Assessment and referral to community-based providers
· Ensure accurate and timely documentation of all relevant client documentation into Veoci database
· Attend and actively participate in regular Project Longevity meetings, reporting and receiving weekly updates.
· Monthly reporting to Justice Education Center on activities of CC as well as notable achievements of PL clients.
· Coordination of efforts with Bridgeport Reentry Welcome Center Staff.
· Must work collaboratively with Project Longevity staff as well as law enforcement, including Probation and Parole.
· Responsible for tracking the use of basic needs funds.
· Travel to and from meetings both within and outside Greater Bridgeport.
Qualifications:
· Associate degree in criminal justice or related field.
· 3 years case management experience.
· Background and understanding of issues related to prison reentry and justice impacted individuals.
· Demonstrated ability to work collaboratively in a team setting and independently manage a caseload with minimal supervision.
· Must have a valid drivers license and access to reliable transportation.
· Proficient with Microsoft Office suite.
· High level of computer literacy and adaptability.
· Proven ability to be a self-starter and effectively prioritize tasks.
· Strong problem-solving skills and ability to think critically to address client needs and overcome challenges.
· Excellent written and verbal communication skills.
