Job Summary and Qualifications

The In-Home Services Manager is responsible for the daily operations of in-home services offered through PACE and Private Duty Services. They have direct accountability for the management and direction of staff to ensure services are provided in accordance with all regulatory requirements and the organization’s policies and procedures. The In-Home Services Manager coordinates and collaborates with various teams on patient care activities to enhance quality of care, improve outcomes, and increase efficacy for an identified at risk population by using a team based case management approach. This role focuses on improving the health status and care for individuals with chronic conditions, high risk patients for hospital admission/emergency department use, and those with potentially complex medical, mental health, and psychosocial issues.

What qualifications you will need:

• Required Education: BSN or equivalent (AND with MSN, BA with MSN)
• Preferred Education: Master’s Degree in Nursing or complimentary field (MBA; MHA)
• Required License:

• Must have and maintain current licensure as a Registered Nurse with the North Carolina Board of Nursing; eNLC license may apply, licensee should confirm with NCBON
• Valid North Carolina Driver’s License
• BLS Healthcare Provider Certification
• First Aid

• Required Experience: A minimum of 3 years recent nursing experience; a minimum of 1-year experience in healthcare leadership
• Preferred Experience: Recent experience working with medically frail adults or elderly population. Working knowledge of physical, mental, spiritual, transportation, and social needs of the frail/elderly and their families. Relevant homecare/home health experience. Experience working effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage participants' care.
