Career Description Summary: The Clinical Coordinator is responsible for every aspect of the day-to-day operations of the Hospice, including all aspects of patient care. The Clinical Coordinator is a member of the Interdisciplinary Team and provides leadership and management of the Interdisciplinary Team to provide coordination of care and to ensure continuous assessment of each patient’s and family’s needs and implementation of the interdisciplinary plan of care.
Essential Functions & Responsibilities:
- Coordinates and directs the agency’s daily workload ensuring appropriate assignments of referrals for all services.
- Coordinates 24-hour availability of required services.
- Assures service satisfaction through patient visits and other measures, as appropriate.
- Investigates and resolves all reported incidents and/or complaints.
- Communicates as appropriate with staff, physicians, referral sources and patients.
- Listens to all customers and maintains an effective working relationship with referral sources responding in a timely manner to their needs.
- Maintains an Interdisciplinary Team/Care Plan schedule monthly and directs all Interdisciplinary Team conferences.
- Works with education staff to provide monthly in-service programs to staff.
- Assists in the supervision maintenance of medical records assuring accuracy, completeness, and compliance with agency policy, regulations, licensures, and certification standards.
- Assists with the recruitment, hiring, orienting, evaluating, and counseling staff.
- Establishes and maintains positive relationships with staff and in the community.
- Participates in short and long-range planning and development and implementation of the agency budget.
- Ensures that quality medical supplies, equipment, oxygen, and medications are available for patients when needed.
- Practices infection control and universal precautions to ensure the safety of the patient.
- Recognizes and reports any form of abuse taking appropriate action according to policy and federal/state laws.
- Lives the PLUS Standards in the care of patients, daily tasks, and interactions with others.
- Maintains regular attendance and punctuality which are critical in order to complete the day-to-day tasks of this position.
Marginal Job Functions:
- Makes patient visits as necessary.
- Participates in agency and community programs as requested to promote professional growth and understanding of hospice care.
- Assists the Executive Team with special projects as requested.
Education and Experience Requirements: BSN preferred. Must have an active RN license in good standing with the state of Oklahoma. One year of supervisory experience and a minimum of two years hospice preferred. Must understand the grief process both in dealing with patients and their families.Must have a valid driver’s license and proof of insurance.
The Clinical Coordinator position is a registered nurse appointed and approved by the hospice governing body and employed by the hospice as a patient care coordinator to supervise and coordinate the palliative and supportive care for patients and families provided by the hospice team.
Essential Physical Requirements: Maximum physical exertion with moderate to maximum mental exertion. Must be able to bend, stoop, climb stairs, reach and turn. Must be able to make visits to patients in a variety of settings. Must be able to lift and carry patient care supplies and equipment up to40 pounds with no ill effects. Must be able to drive for extended periods of time, both at night and during the day.
Working Conditions: Constant contact with families, visitors, patients, and employees. Will work in patient homes, other healthcare facilities, and offices.
