Case Manager
Community Aging and Retirement Services, Inc. is currently seeking FULL TIME CASE MANAGEMENT STAFF. The case manager acts as the community care service system gatekeeper with the knowledge and responsibility to link clients’ needs to the most beneficial and least restrictive array of community services and resources.
This is a full-time billable position which includes:
· Travel to/from clients’ homes within Pasco County
· In-home assessment/reassessment of client need
· Individualized care plan development
· Arrange and evaluate services provision.
· Complete detailed progress notes meeting all billable requirements.
· Identify, coordinate and link community resources/agencies.
Requirements:
Bachelor’s Degree in Social Work, Gerontology, or related Human Services field or AA Degree with 2 years Case Management experience required.
· Organized and detailed oriented
· Sense of urgency and know-how to prioritize.
· Multi-task with strong computer, written and verbal communication skills.
· Work efficiently under deadline.
· Working knowledge of community organizations for the elderly (Preferred)
· Must pass a level II background screening, have reliable transportation, a cell phone with a data plan, and pass a drug screening.

Pay range $20-$22 with applicable experience plus mileage for travel throughout Pasco County
