We need a Regional MDS Coordinator to join our team at Carespring Healthcare Management in Loveland, OH.You will travel to our Ohio and Kentucky facilities assessing our new/existing patients to ensure all of the care needs are addressed and documented on the MDS. Must be a licensed RN or LPN in Ohio with prior experience in a long-term health care setting $28 to $44 Per Hour You'll Be Responsible For:
Training new MDS Coordinators/MDS Nurses to conduct resident assessments, develop plans of care, evaluate residents' responses to interventions, document clinical records, RAI manual and all applicable deadlines for resident assessments, and complete Minimum Data Sets (MDSs)
Serving as RN Resident Assessment Coordinator (RAC) or MDS Nurse when needed
Assisting with MDS Completion as needed
Providing comprehensive training for systems related to the patient assessment instrument, clinical software, Medicare reimbursement, Medicare coverage/eligibility requirements, State Medicaid reimbursement systems, and special contract systems
Monitoring RAI process for accuracy and completeness
A working knowledge of PDPM and Case Mix in the state where the facility is located
A working knowledge of facility Admission process and assists with admissions as needed
Assisting facilities with surveys and survey preparedness
Maintaining ability to obtain and review QI/QM's, working with IDT to initiate improvement projects to improve resident outcomes
Attending IDT Medicare meetings, PDPMs, and Medicaid meetings to collaborate with the IDT to complete the MDS process
Reviewing to ensure accuracy of supportive documentation according to MDS/RAI regulations
Traveling to various facilities
Educating licensed and non-licensed staff
You'll Need To Have:
A current LPN or RN license in Ohio and/or Kentucky
3 - 5 years MDS/RAI experience preferred
Flexibility with your work schedule and be able to work independently
Excellent professionalism and organization skills
Solid communication and teaching/training skills
The ability to positively interact and communicate with the leadership team in multiple facilities
An understanding of the MDS process along with a willingness to continue to learn and remain current with MDS updates set by CMS
A genuine desire to provide consultation, guidance, support, research, analysis, training, and expertise in the delivery of care and services to our patients regardless of payer source
The willingness to obtain/maintain a current MDS Certification within 60 days of hire
