The Licensed Practical Nurse (LPN) assumes responsibilities for direct nursing care of assigned residents under the supervision of a registered nurse (RN). Provides nursing services to residents and families in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing.
- Provides direct nursing care to residents under the supervision of a registered nurse. Nursing care is guided by the provider orders and the nursing plan of care within the LPN scope of practice.
- Reports resident response to care to registered nurse or other provider for evaluation, intervention and modification of the plan of care.
- Ensures documentation of care is complete and accurate.
- Responsible for relaying result messages to residents over the telephone.
- Responsible for managing refill requests for residents.
- Administration of medications and immunizations for residents.
- Ensures adherence to established standards of care and scope of practice.
- Ensures documentation of care is complete and accurate.
