Carobell, Inc. is an intermediate care facility, day program and care management provider for individuals with moderate to profound developmental disabilities. We offer many services to the community from residential care, to day vocational programs, to entrepreneurship, and care management services, which is where you come in. We are currently seeking a professional with experience working in a supervisory role in care/case management and/or social services.
A summary of the job description:
The Care Management Supervisor will be responsible for providing clinical, administrative, and operational supervision and guidance to the Care Management team to promote whole person, integrated care. He/she will regularly communicate and collaborate with the care management team and the Tailored Care Management Director to support Carobell, Inc's. mission and ensure quality of service delivery.
Successful candidates must hold a bachelor’s degree in a social service field, have at least five year’s experience working directly with the MR/IDD, mental health and/or substance use disorder population, be team oriented, have a valid drivers license and must pass a drug screen and criminal background check.
