  • JOB RELATIONSHIPS
    • Responsible to the Assistant Director and Director of Care Coordination and Community Health Programs.
  • DEFINITION OF POSITION
    • The Medical Social Worker is a Masters or Bachelors prepared individual who is responsible for facilitating social work services to patients and their families, who utilize both inpatient and outpatient medical services, in conjunction with interdisciplinary health care team members. The Social Worker is a member of the hospice interdisciplinary team (IDT) whose primary function is to provide psychosocial support to the patient and family as they travel the end of life journey. The social worker is also a member of the Home Health team and serves the primary function to support patients and resolve social or emotional problems that are, or may be impediments to the patient's recovery. 
      Social worker will conduct initial and ongoing assessments to identify the needs, goals, interventions and services needed and then work with the IDT to create a plan of care that promotes quality of life, comfort and peace. Services include advising patients and family caregivers of community resources and supports, helping to guide individuals through the process as needed. The social worker will provide education where appropriate, empowering patients to share in the decision-making process when determining their options for treatment and/or need to access services appropriate to meet their individual health needs.
  • QUALIFICATIONS
    • Professional
      • Master of Social Work (MSW) degree from a program accredited by the Council on Social Work Education preferred or a Baccalaureate degree in SW(BSW) accredited by CSWE.
      • Two to three years of post-masters experience in a health care setting such as a hospital, mental health setting, extended care facility, or home care agency is desirable (1 year required).
      • Knowledge of applicable legal responsibilities/restrictions, current trends, and end of life issues in social work pertinent to the home health/hospice practice setting.
      • Knowledge of acute, chronic and terminal illnesses, mental and physical disabilities, and the normal developmental process and the impact on family systems.
      • Evidence of participation in ongoing continuing education programs.
      • Knowledge of and adherence to the National Association of Social Workers Code of Ethics.
      • Must be willing to travel, maintain a valid North Carolina driver’s license, auto liability insurance, and dependable transportation.
