Cascade AIDS Project (CAP) is the oldest and largest AIDS Service Organization (ASO) in Oregon and Southwest Washington, as well as a Federally Qualified Health Center (Look-A-Like) that provides the LGBTQ community and beyond with compassionate healthcare. The Our House Residential Care Facility (RCF) is a 14 room facility with a team of expert clinicians who work with people living with HIV who are having difficulty managing independent living. By tailoring nursing, social services, and therapeutic everyday activities our clients and residents are able to define what ‘living well’ means to them.

 

 

 

CAP and Our House is excited to announce that we are hiring an experienced NHCP Outreach Case Worker.

The Outreach Case Worker will be instrumental in the recruitment of new clients for NHCP. This position is split between the needs of program outreach via developing community relationships and connecting with new clients, and case management via working collaboratively with team members (Peer Recovery Mentors, Case Workers, and Registered Nurse) to provide individuals who are living with HIV, Substance Use Disorders, and Mental Health conditions with wrap around supports so they can live as independently and successfully as possible in their community.

NOTE: This is a non-management, union-represented position. The compensation, benefits, and conditions of work for this position are collectively bargained. 

 

 

COMPENSATION: $25.04 hr

 

Successful candidates will have:

  • Representative of the knowledge, skill, and/or ability required.
  • Prior experience working with people in the field who have substance use disorders and/or Mental Health conditions.
  • Experience working in outreach or recruitment setting and demonstrates skills in networking.

 

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree in social services or a related field.
  • A valid driver’s license, acceptable driving record, and ability to be insured by Our House’s auto insurer.

 

PREFERRED QUALIFICATIONS:

  • Ability and comfort level to work with the needs of those that are houseless.
  • Ability to respond to ever-changing environments and utilize strong decision-making skills.
  • A strong understanding of community resources.
  • Ability to work well independently and as part of a team.
  • Ability to cultivate and maintain professional relationships across a multicultural spectrum of organizations and professionals, including community partners and funders.
  • Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance use issues.
  • A strong understanding of issues relevant to people living with HIV, communities of color, and the LBGTQ community and how those issues affect or influence service delivery.
  • Demonstrated commitment to advancing equity and inclusion in workplace or community settings.
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to: engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities that we serve.
  • Experience working with people living with HIV/AIDS (PLWHA) and/or people with disabilities, chronic illness and/or other barriers to employment.

EMPLOYEE BENEFITS:

CAP is proud to offer a comprehensive benefits package for eligible employees.

Any non-temporary full-time or part-time employee (working .5FTE or greater) is considered eligible for the benefits. Benefits may be adjusted based on FTE or hours worked. The below benefits are for 1.0 FTE or 40 hours a week.


Vacation Leave Other

  • 16 days of paid vacation (128 hours) each year for the first two years of employment
  • 21 days of paid vacation (168 hours) each year of employment beginning in year three
  • 12 days of paid health leave per year
  • 12.4 paid holidays per year
  • Longevity bonus – two weeks of paid leave on the fourth anniversary, must be used within two years after the date awarded. Additional longevity bonuses are awarded at eight-year and 12-year anniversaries.

CAP cares about the safety of our team members and clients. All new employees are required to provide proof of vaccination status to HR at the time of hire (unless a documented medical or religious exemption is requested and approved).
