Cascade AIDS Project (CAP) is the oldest and largest AIDS Service Organization (ASO) in Oregon and Southwest Washington, as well as a Federally Qualified Health Center (Look-A-Like) that provides the LGBTQ community and beyond with compassionate healthcare. CAP’s mission is to promote well-being and advance equity by providing inclusive health and wellness services for LGBTQ people, people affected by HIV, and all those seeking compassionate care. Our House provides integrated health and housing services to people with HIV. Guided by compassion, we provide 24-hour specialized care, supportive services, and independent housing support services. Prism Health is the only nonprofit LGBTQIA health center providing comprehensive primary care, behavioral healthcare, and pharmacy services in the Pacific Northwest (Oregon, Washington, and Idaho). Since opening its doors in 2017, Prism Health has become a model for culturally specific care and now serves over 1,500 patients with a wide range of services, from day-to-day primary care and pharmacy needs to targeted services like gender-affirming care and specialized population health screenings.

 

 

 

Prism Health is excited to announce that we are hiring an experienced Outreach Worker/QMHA for Trans / Gender Diverse Services. The Outreach Worker/QMHA provides services to transgender identified individuals enrolled in Primary Care or Behavioral Health services who have been identified as having case management needs. The Outreach Worker/QMHA works to connect individuals to resources, promote greater independence, community integration, and recovery. This position will work collaboratively with the Peer Support Specialist to help resolve barriers and connect individuals with resources in the community.

The person in this position will be utilized between Prism Health and Providence Health & Services. This position includes travel throughout the service area for meetings and client home-visits. Must be available to work evenings and weekends for required agency events.

This position is stationed in CAP’s Prism Belmont office. Occasional evening and weekend work is required.

 

ESSENTIAL JOB FUNCTIONS:

  • Work in collaboration with Prism Behavioral Health team, Providence Health & Services, medical providers, service navigators, or other providers involved in client care.
  • Uses a Person-Centered approach to create and work on treatment goals tailored to the specific needs of the individual receiving services.
  • Uses own experiences as guided by training and models the principles of recovery for those they work with, including gaining greater independence, emphasize wellness and self-determination.
  • Accompany clients to appointments to facilitate access to medical care, substance use treatment, mental health services, housing, and other services.
  • Assists the persons they work with to identify and utilize natural community supports to decrease dependence on the traditional mental health system.
  • Provide support to clients who are having symptoms that could result in hospitalization. Duties could include grocery shopping, meal preparation or just spending time with clients until they feel secure enough to be on their own.
  • Provides client assistance and support to connect with community resources, as well as life skills development and self-sufficiency coaching.
  • Work with clients on specific and obtainable goals included in the person's treatment plan.
  • Provide accurate, complete, timely and current written and database records/files, including timely data entry.
  • Meet all contract requirements and provide supporting documentation including program performance data and reports as required.
  • Provide regular updates to medical case managers and other service providers as to the progress of each client towards achievement of their goals
  • Design and produce written materials in support of the program, either as an individual or as a member of the team.
  • Assist in the development of culturally appropriate programs and the recruitment of clients for those programs.
  • Work collaboratively with and communicate effectively with CAP/Prism/Providence staff, volunteers, community partners, and clients.
  • Attend training and educational opportunities to remain current on HIV, case management, and other relevant professional issues.
  • Participate in appropriate community, department, and agency meetings as assigned.
  • Assist in the development and implementation of equity and inclusion initiatives within the departments, both at CAP/PRISM and Providence Health & Services.
  • Participates and engages in equity-based trainings, workshops, and team discussions.
  • Support the implementation of CAP and Providence’s organizational equity initiatives.
  • Participate in Peer-Focused Support and Oversight with Providence BOB/Peer Team.
  • Other duties as assigned.

 

NOTE: This is a non-management, union-represented position. The compensation, benefits, and conditions of work for this position are collectively bargained. 

 

COMPENSATION: $52,083.20 annually

 

MINIMUM QUALIFICATIONS:

  • QMHA credential through MHACBO
  • Bachelor’s degree in behavioral sciences field (social work, public or community health, psychology) or one year of relevant work, education, training, or experience.
  • Knowledgeable about the lived experience of experiencing mental health and/or substance use issues and willingness to mindfully share those experiences.
  • Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance abuse.
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities we serve.
  • Basic computer and keyboard proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet.
  • Demonstrated written and verbal communication skills.
  • Demonstrated organizational and time management skills.
  • Able to travel throughout the service area on a regular basis.
  • Ability to work independently or within a team with accountability; exercise sound judgment and discretion, maintain healthy boundaries, respect, discretion, and professionalism always.
  • Demonstrated problem-solving skills and familiarity with de-escalation techniques.
  • On occasion, be available to work evenings and weekends for required agency events.

 

PREFERRED QUALIFICATIONS:

  • CADC through MHACBO
  • Verbal and written fluency in English and Spanish.
  • Experience working with OCHIN Epic electronic health record (EHR).
  • Demonstrated commitment to advancing equity and inclusion in workplace or community settings.

Any non-temporary full-time or part-time employee (working .5FTE or greater) is considered eligible for the benefits. Benefits may be adjusted based on FTE or hours worked. The below benefits are for 1.0 FTE or 40 hours a week.
