POSITION SUMMARY
The Resident Care Coordinator (RCC) helps create a joyful and vibrant community by participating in the orientation and training of staff as assigned by community leadership and serving as a mentor and resource to team members.
ESSENTIAL JOB FUNCTIONS
- Act as a lead/trainer.
- Acts as on-call staff and fills in shifts when requested by the Director of Nursing Services.
- Reviews assignments with Executive Director and/or Director of Nursing Services.
- Reads staff communication records and maintains familiarity with resident records as required.
- Assists residents with activities of daily living, assistance with medications, treatments and other care while encouraging self-care and independence.
- Provides resident services as indicated on the resident’s assessment and service plan. (This includes bathing, grooming, medication, and providing assistance or administrative duties related to daily activities/living.
- Observes problems, concerns, issues with employees and/or residents and communicates them appropriately to Executive Director and Director of Nursing Services.
- Records resident information as required.
- Assists with the orientation of new staff members to job responsibilities.
- Checks daily for physician orders and follows up if order is not returned.
- Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security work, safety, and other emergency procedures.
- Notifies physician, nurse and / or designate agent(s) of resident status/change as appropriate.
- Coordinates care with outside agencies.
- Conducts service plan meetings with residents and families.
- Ability to work a flexible schedule, including weekends, holidays and overtime, to meet the requirements of the position.
- Ability to relate positively and communicate appropriately with residents, families, and employees.
- Has compassion and special interest in working with the senior population.
- Ensures all medications have been re-ordered and residents do not run out of Medications.
- Ensures all doctor’s orders have been transcribed correctly in the MAR (Medication Assistance Record) and ensures all orders have been signed off by the Medication Aide.
- Completes monthly MAR reviews and follows up on any discrepancies.
EXPERIENCE, QUALIFICATIONS & SKILLS
- Licensed caregiver
- A high school diploma or equivalent.
- Ability to read, write, speak and understand English.
- Meets all mandatory health and training requirements by State regulations.
- Maintains a current First Aid and CPR.
- Pass a Criminal background and fingerprint check.
- Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
- Has compassion and a special interest in working with the senior population.
- Ability to work a flexible schedule, including weekends, holidays and overtime, to meet the requirements of the position.
- Ability to relate positively and communicate appropriately with residents, families, community members, volunteers and other employees.
- Ability to creatively problem-solve in resident care
- Maintains clean, neat, comfortable, safe environment for residents, staff, and visitors.
- Ability to work independently (with minimum supervision)
- Maintains current Food Handlers Card.

Actual pay rate will be calculated based on relevant years of experience and education/certification requirements.
