Description

Coordinate and identify services that will provide eligible clients the ability to develop skills that will increase access to and retention in medical care; and establish the foundation that will allow them to achieve and maintain stable, independent housing within Monroe and surrounding Finger Lakes Counties.

 

SPECIFIC DUTIES:

  1. Conduct assessments and planning to address housing needs, when appropriate complete intake, assessment, service plan development, implementation and referrals.
  2. Assist needed clients in locating appropriate and affordable housing and oversee implementation of plans to the satisfaction of the individuals served.
  3. Serve as client advocate with landlords, housing programs and other organizations client is affiliated with.
  4. Maintain and identify new landlord or property owner resources.
  5. Assist clients in completing housing or emergency assistance applications, DHS application and other paperwork need to complete service plan development.
  6. Participate in case conferences, provider meetings or housing committee meetings that are pertinent to clients needs.
  7. Complete required home visits with clients and maintain monthly contact as directed by supervisor.
  8. Complete housing inspections annually and as needed by funder.
  9. Ensure compliance with all applicable local, State and Federal regulations and agency policies.
  10. Meet with each client and maintain telephone availability for consumers based on client need
  11. Assist clients with accessing services for housing, medical services, and other psycho-social needs make appropriate referrals when needed.
  12. Assist clients with obtaining and maintaining all benefits and entitlements to ensure ability for payment of housing and becoming self-sufficient.
  13. Assist clients with Independent living skills, such as budgeting, banking, and child care referrals in order for them to maintain appropriate housing.
  14. Ensure all required information is entered in to all databases systems used.
  15. Assist clients with obtaining house hold items.
  16. Assist client with follow through of all requirements of the HomeSOURCE, HOPWA and Ryan White programs.
  17. Travel to rural counties to assist clients in the Finger Lakes Region, if applicable.
  18. Market HomeSOURCE, HOPWA and Ryan White programs.
  19. Document all communication with or regarding clients in detailed progress notes.
  20. Maintain timely case openings, case management logs, case closings and all other necessary and required paperwork.
  21. Provide crisis intervention.
  22. Attend mandatory training.
  23. Act as a member of HomeSOURCE, HOPWA and Ryan White team.
  24. Represent Catholic Charities Community Services on appropriate boards and committees as assigned by supervisor.
  25. Any other duties directly assigned by supervisor.
  26. Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CCDRs corporate compliance and ethics program.
  27. Possess excellent verbal and written communication skills.
  28. Willing to travel to rural counties served by Housing Program.
  29. Experience and knowledge regarding Benefit and Entitlement Programs (DHS, SSI/SSD).
  30. Previous experience working with diverse populations, including low to moderate-income families.
  31. Ability to multi-task and prioritize duties.
  32. Ability to travel in all weather conditions throughout the counties served.
  33. Willingness to foster agency, department and program wide cooperation and team work through use of positive/constructive communication techniques.
  34. Ability to maintain absolute confidentiality in regards to all records reviewed including client records, employee records and billing records.
  35. Proficiency and experience with PCs and Microsoft applications.
  36. Ability to analyze and interpret data and to handle problem resolution.
  37. Possession a valid NYS Drivers license and a driver's record considered acceptable by agency and insurance carrier.
  38. Continuous use of a reliable, registered and insured vehicle.
  39. Demonstrate commitment to Agency Mission Statement.
  40. Provide effective, equitable, understandable and respectful quality care and services that are responsive to diverse cultural health beliefs and practices, preferred languages, health literacy and other communication needs.

Qualifications


Bachelors degree in Health, Education, or Human Services and at least two years of experience with HIV/AIDS, chemical dependency, or mental illness.

OR

Associates degree in Health or Human Services and at least 3 years experience with HIV/AIDS chemical dependency, or mental illness.

A combination of education and experience will be considered at the discretion of the Director.

Bilingual (English/Spanish) is required for some positions. If position requires bilingual skills to be used, additional compensation will be offered.

 

Ability to meet the following physical requirements with or without reasonable accommodation:

  • Use hands to manipulate, handle, feel, and control items or equipment, including motor vehicle;
  • Stand, sit, walk, bend, stoop, kneel, and reach;
  • Climb up or down stairs;
  • Able to reach above or below shoulders;
  • Occasionally lift or move objects weighing up to 10 lbs;
  • Sitting at a desk or in a vehicle for long periods of time to perform certain job functions;
  • Be able to read write and interpret written documents;
