This position provides direct case management services to the poor and the disenfranchised through advocacy, information and referral, crisis intervention services, and short-term case management.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
1. Provides compassionate and timely crisis intervention to individuals and families, facing emergencies around food, shelter, medical needs, and any concrete need as presented by the client
2. Conducts thorough needs assessment and develops a service plan to address identified concrete needs using short-term intervention model.
3. Conducts psychosocial assessments of individual and family problems and follow up with referrals to Catholic Charities programs and/or clinics or an outside facility as deemed appropriate.
4. Maintains current and accurate documentation of services provided to clients.
5. Processes EFSP grants by obtaining supporting eligibility documentation.
6. Provides short-term case management focusing on assessing factors precipitating housing crisis.
7. Develops and implements preventive measures to reduce risk of eviction in the immediate future.
8. Maintains current and accurate documentation of services provided to clients in agency-administered database. This includes completing statistical reports on a timely basis
9. Attends and actively participate in supervision sessions with immediate supervisor.
10. Attends agency’s in-service training, department meetings, in addition to other agency-related activities (i.e. external network meetings, resource exchange, SALT)
11. Develops and maintains good communication with other programs and departments within the organization to facilitate inter-agency referrals. Develops contacts in the community for referral purposes.
12. Provides case management coverage throughout the county and other locations as needed.
13. Any other duties assigned by Supervisor
Position Type and Expected Hours of Work: This is a full-time position. Days and hours of work are generally 9:00 AM to 5:00 PM Monday - Friday. Additional hours during evening may be required to attend events and meet Agency and program needs.
Working conditions and physical demands required: The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
Remain in a stationary position at a work station and use a computer at least 70% of the time.
