LEAD Coordinator

We are currently hiring a LEAD Coordinator, to work in our Care Coordination Services Office. As the LEAD Coordinator, you would be responsible for:

• Address program vacancies or expansion by conducting interviews, obtaining references and completing other assigned tasks related to the hiring process.
• Provide orientation and training to all new and existing staff as needed.
• Facilitate general program oversight and development.
• Monitor the oversight of all outreach and referral services provided through LEAD
• Conduct initial screening and/or intake activities for diverted/referred participants
• On Call phone rotation once every six weeks

LEAD Coordinator* Minimum Salary: $58,500
LEAD Coordinator Maximum Salary: $58,500
LEAD Coordinator Schedule: Mon-Fri 8:30am-4:30pm (some evenings and weekends as required)

Qualifications:*

• Associates or Bachelors Degree in health, education or human services OR RN, LPN, CASAC
• At least one year of supervisory experience of outreach, Case Management or case work
• 1- 3 years’ experience with persons with: HIV/AIDS or other chronic conditions, a history of mental illness, homelessness, chemical dependence, and/or other populations of persons in need-one year of which must include supervisory experience/responsibilities
