As the Wellness Advocate, you will play a pivotal role in individuals' lives, making a significant impact through:
- Conduct outreach and engagement activities in greater capital region based on member lists provided by CDPHP
- Represent the agency in keeping with agency mission and standards of professionalism and ethics
- With supervisory approval, register for and attend community trainings/workshops to comply with agency and other requirements
- Manually and/or electronically document activities and patient/member information and interventions as directed
- Communicate to supervisor of any site-specific needs for repair or equipment
Wellness Advocate Minimum Salary: $19.49
Wellness Advocate Maximum Salary: $19.49
Wellness Advocate Schedule: Monday-Friday
Qualifications:
- Bachelor's degree in the following or related fields: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreation therapy, rehabilitation, social work, sociology, or speech and hearing;
- OR NYS licensure and current registration as a Registered Nurse and a bachelor’s degree;
- OR A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses;
- OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
- : Preferred Licensed level clinician with prior experience in a behavioral health clinician or care management supervisory capacity; Working knowledge of the NYS Health Home Program.
- 2 years experience working with a diverse population
