Responsibilities include:

Communicates with Medical Director and Admissions Coordinator regarding potential candidates for inpatient rehab unit.

Communicates with RN manager and Director of Therapeutic Services regarding any special needs of accepted patients to ensure that they can be appropriately accommodated.

Communicates with rehab Care Management team regarding any discharge disposition concerns.

Completes pre admission screen in a timely manner.

Maintains communication with referring facilities including facility visits.

Participates in vendor fairs at referring facilities.

Assists with marketing initiatives to grow rehab program.
Qualifications:
-
Graduate of an accredited school of nursing, social work, Physical Therapy, Occupational Therapy, or Speech Language Pathology; minimum BS

NYS license in Social Work, Physical Therapy, Occupational Therapy or Speech Language Pathology

Five years of clinical experience in the acute rehabilitation setting; excellent interpersonal and verbal/written communication skills, a strong sense of professionalism and ethics, strong presentation skills
