Registered Nurse (RN) - Hospice Liaison

This position will be based out of St. Charles Hospital in Port Jefferson.

Our Hospice Liaison is responsible for assessing, planning and developing referrals. This position is located at Stony Brook Hospital in Stony Brook, NY. This position will establish and maintain professional relationships with key physician groups and referral sources, as well as act as a liaison to referral sources on behalf of Good Shepherd Hospice and identify potential opportunities for market growth and program expansion.

The ideal candidate will possess excellent interpersonal, marketing and customer service skills, and act in a professional and supportive manner at all times.

Minimum Requirements:

• Current and valid NY State license to practice as a Registered Nurse

• Acute care hospital experience required

• Proficient with Electronic Medical Records

• Two years hospice experience preferred

• Bachelor's degree preferred

The pay for this position ranges from $90,000 – 113,000 per per year.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
