What are the responsibilities and job description for the RN Navigator position at Catholic Health?
Catholic Health Pregnancy Support Program Goal:
On May 12, 2022, The New York State Catholic Conference (NYSCC) issued a statement by the Catholic Bishops of New York State, emphasizing a Pro-Life future and need to support pregnant women. As the Catholic Health system on Long Island, it is our mission to better integrate our services and expand access for women in need. This position will serve as the RN per diem navigator to expedite care for women at risk of abortion, provide counseling, and connect with community organizations that offer social supports.
Responsibilities:
- Assist with the development of patient flows to expedite care for women at risk of an abortion
- Acts as a source of information for the patient and articulately responds to questions.
- Provides resources and education about pregnancy and connects as appropriate to community-based organizations for social supports.
- Supports the participation of patients in planning for their care
- Occasionally performs scheduling of office visits and procedures.
- Provides educational materials to patients in a timely manner.
- Serve as the liaison with the hospital services, community organizations and the Rockville Centre Diocese.
- Involved with community outreach and other marketing efforts in support of the program.
- Upon consent, supports the communication between the patient’s OB/GYN and/or primary care physicians, or other referral source.
- Demonstrates appropriate use of independent judgment and decision making
- Collaborates with all members of the health care team
- Makes appropriate referrals to resolve patient issues effectively
- Actively empathizes with patient, expresses compassion and understanding for the situation and offers support/encouragement where appropriate
- Demonstrates sensitivity in communications considering the age and cultural, religious or ethnic background of the patient
- Effectively utilizes the computer to transcribe orders or request printouts
- Acts as an effective liaison to medical staff to ensure continuity and congruity of hospital services and other settings
- Participates in professional development
- Ensures compliance with hospital wide standards and follows policies and procedures.
- While the primary location is Babylon , NY there may be travel to other Catholic Health facilities.
- Performs other duties as required in support of Gianna Centers.
Required Qualifications:
- Current New York Registered Nurse license required.
- Five years’ experience in a clinical setting with an interactive role with patients.
- Knowledge of labor, delivery, postpartum and nursery care essential.
- Must know principals and practice of sterile technique and infection control.
- Strong alignment with the Ethical and Religious Directives
- Ability to work effectively with colleagues, physicians and others as part of a care team
- Ability to maintain flexibility and work well in a constantly changing environment
- Ability to maintain attendance standards of the organization to support organizational goals.
- Ability to facilitate conversations regarding pro-life options
Preferred Qualifications:
- Experience with the outpatient care related to women’s health
- Understanding of The Joint Commission requirements Skilled with Microsoft Office and Electronic Medical Records
- Active member of AAFCP
- Certified as an Allied Health Care Professional from Pope Paul VI Institute.
