Job Details
Qualifications •Nursing Home experience preferably •MSW Required & Must be licensed by the NYS. | Responsibilities •Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs •Initiating appropriate interventions for residents experiencing mental or behavioral health issues •Identifying individual social and emotional needs •Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility •Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay •Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community |
Salary Range
USD $41.31 - USD $41.31 /Hr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
