RN Field Home Care Supervisor - PRN

POSITION SCOPE: The Home Care Supervisor is accountable for ensuring the overall quality and appropriateness of patient care and nursing services for all home care clients as assigned.

POSITION QUALIFICATIONS:

  1. Currently licensed as a Registered Nurse in Rhode Island with an unencumbered license
  2. Minimum of three years nursing experience.
  3. Minimum of one year supervisory nursing experience preferred.
  4. Evidence of annual TB tests or screenings and other exams as required by law.
  5. Strong interpersonal skills.
  6. Strong communication and problem-solving abilities.

DUTIES AND RESPONSIBILITIES:

  1. Supervision: Utilization of appropriate interpersonal styles and methods in guiding staff members

toward company objectives.

Evaluate patients referred for home care to determine appropriateness for admission. Refer patients needing other resources of care to appropriate services. Consult with family, physicians, institutional staff, clients, nurses, and other disciplines while establishing a coordinated home care plan.

Notify appropriate branch personnel of patient acceptance for home care and provide

intake data, essential background information, care plan, and signed physician orders. d. Oversee the initial and ongoing assessment of home care clients. e. Complete a nursing assessment of all home care clients within 48 hours of order. f. Initiate, review, and revise the plan of care. g. Obtain a physician's plan of treatment/care and ensure their execution. h. Review plan of care every 90 days or more often, if necessary. i. Review timesheets bi-weekly and ensure that tasks match the care plan. j. Visit all home care clients as required by Hope Nursing Home Care to:

  1. Assess the patient's overall health status and home safety environment.
  2. Review and determine that care is provided according to the care plan.
  3. Determine care is appropriate to identified needs of the patient.
  4. Revise the care plan as needed.
  5. Determine care is being provided on assigned days and time as scheduled.
  6. Determine that staff member is oriented to assigned responsibilities and the

patient's individual needs. Ensure that competency screening is initiated and on-going with supervision.

  1. Review bloodborne pathogens and exposure control with each employee

during orientation and review on an annual basis.

  1. Ensure that the client is satisfied with service, k. Handle and report problems to appropriate parties.
  2. Confer with agency personnel on client progress and ongoing evaluation. m. Evaluate all employees as required by Hope Nursing Home Care standards. n. Counsel employees as needed. 0. Any other duties as assigned.
  3. Operations/Administration: Coordinating the various aspects of branch operations required to

ensure compliance with all federal, state, and local government laws and guidelines as well as Hope Nursing Home Care policies and procedures.

  1. Responsible for implementing, maintaining, and following all appropriate standards.
  2. Participate on an assigned advisory board or utilization review committee.
  3. Review all client records every 90 days or more often, if necessary.
  4. Verbal or written report to physician as necessary regarding client's response to services.
  5. Secure/coordinate equipment and supplies for the client as needed in the home.
  6. Assist staff in determining skills, specialties, and preferences of employees.
  7. Assist with orientation of new employees and review blood borne pathogens, exposure

control sections and other pre-competency screenings with each new employee.

  1. Give input and review skill matching of employee's competency to client needs.
  2. On-Call duties
  3. QA checks on 10% of clients' charts of office every quarter per policy.
  4. Community/Client Education:
  5. Develop and maintain working relationships with key contacts in health care institutions and community service organizations.
  6. Communicate the concept of quality client care by Hope Nursing Home Care to associates in various settings. d. Provide consultation on discharge from care or re-admission to hospital or other facility. e. Provide feedback to

facility personnel on referrals.

  1. Serve as a resource to the community on home care issues and practices.
  2. Participate in speaking engagements. g. Call on physicians, discharge planners, trust officers, case managers, and other contacts on a regular basis to report quality home care services being provided to clients by agency. Educate professional and lay individuals about the services Hope Nursing Home Care provides.
  3. Provide new and/or continued service by cultivating and establishing rapport with

members of the healthcare field.

  1. Responsibilities:
  2. Maintain current RN license. b. Obtain annual TB screening and other health exams as required by law.

  1. Adhere to Hope Nursing Home Care Code of Ethics, the Standard of Practice, and the

Statement of Rights and Responsibilities.

  1. Maintain awareness of development in clinical nursing.
  2. Be familiar with current legislation.
  3. Adhere to Hope Nursing Home Care policies and procedures.
  4. Maintain office in compliance with all state and federal guidelines.
  5. Planning and Organizing: Establish a course of action to accomplish specific goals. Plan proper assignments of personnel and appropriate allocation of resources. Establishing goals, budgeting, setting priorities, allocating proper amounts of time to activities, and maintaining an awareness of inter-relationships between activities.
