CCC’s Hansen Shelter Clinic is seeking a Clinic RN! 

Schedule: Monday, Wednesday, Friday 8:00am-5:00pm at Hansen Clinic; Thursday 8:00am-5:00pm with rotation at our Old Town Clinic or Blackburn locations.    

The Hansen Shelter Clinic RN is responsible for maintaining the smooth clinical operation of the Hansen Shelter Clinic and Health Connections program. The Hansen Shelter Clinic RN works closely with and reports directly to the Director of Nursing and Associate Medical Director of Primary Care. In your capacity as a nurse, the Hansen Shelter Clinic RN provides nursing and clinical support as delegated by Hansen Shelter Clinic providers, as well as supporting navigation, facilitation and coordination of services for Health Connections program participants.

Duties for this role:

Nursing care:

  • Provide regular triage for participants experiencing acute and sub-acute illnesses for the purpose of coordinating care.
  • Provide non-invasive medical treatments, including taking vital signs, performing limited wound dressings, etc.
  • Provide medication monitoring, in partnership with OTC pharmacy, in accordance with participant’s care plan.
  • Provide initial assessment, determine chief complaint, and ensure that patients meet the established criteria for provision of clinical services.
  • Conduct assessments and present findings to provider via remote electronic connection to address acute medical needs of shelter residents.
  • When possible, provide screening and comprehensive assessment of on-going health issues (e.g. HIV, depression, diabetes, asthma, wound and pain management, and other chronic diseases.)
  • Assure completion of referrals and coordinate care with clinical specialists.
  • Recommend changes to medications and medical care plan as needed, change the nursing care plan as progress dictates.

Client Care:

  • Manage patient flow in clinic and rooming patients per standard workflow, including but not limited to:
  • Completion of preventive screening questions.
  • Medication reconciliation with patients.
  • Completing and documenting provider orders.
  • Assisting providers and other team members in scheduling of outside referrals.
  • Administer injectable medications per provider order, with appropriate documentation according to clinic protocols.
  • Maintain medical/lab equipment and records in compliance with State and Federal regulatory requirements.
  • Provide instruction, training, and health information on self-care and health maintenance as per the clients care plan.
  • Respond to emergency events in the shelter.
  • Administer medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, IV medication administration, as well as splinting.

Health Connections Team Lead:

  • Review clinical assessments to determine program eligibility for referred participants.
  • Provide TPI Health Connections and Hansen Shelter staff education and support around basic medical knowledge.
  • Develop relationships and workflows with external primary care clinics to increase understanding of recuperative care continuum and maximize PCP engagement during participants’ respite care stay.
  • Evaluate enrolled participants for medical stability, collaborating with the Hansen Shelter Clinic providers.
  • Participate in and facilitate collaborative multi-disciplinary care coordination, including case conferences and care coordination huddles.
  • Participate in admission and discharge planning for any participant needing hospitalization or subacute level of care.
  • Participate in population health activities, including participant chart and ALERT system reviews to ensure that participants have received appropriate vaccination for influenza and pneumonia.
  • Develop and maintain standardized protocols and programmatic interventions based on most common diagnoses of enrolled participants, in collaboration with participants and relevant care providers.
  • Support community partners in furthering understanding of the medical and psychosocial sequelae of homelessness and the systems and processes used to help complex and medically vulnerable participants secure and maintain stable housing, healthcare, and other social services.

Administrative:

  • Responsible for the oversight of the clinic space, including timely ordering of supplies, recognition and facilitation of equipment maintenance, and ensuring a clean and organized clinical care area.

Minimum Qualifications: 

  • Successful completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board approved course.
  • Current RN license in the State of Oregon.
  • Wound care experience preferred.
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have current BLS certification prior to start date.
  • Must have an understanding of the causes of homelessness and poverty, the medical and psychosocial sequelae of homelessness and the systems and processes used to help complex and medically vulnerable participants secure and maintain stable housing, healthcare, and other social services.
  • Must have experience working with high-risk participants.
  • Must have a high tolerance for ambiguity and change.
  • Must be able to document nursing interventions and responses to care and do so in an accurate, legible, and intelligible manner.
  • Ability to adhere to Central City Concern’s drug-free workplace adopted from Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
