PROGRAM DESCRIPTION:

The South King County Shelters (SKC) provides emergency shelter and case management to single adults who are experiencing homelessness in the South King County Region.  CCS has rapidly expanded services in this area and has taken on a lead role in developing new resources for people who are homeless in South King County. The South King County SeaTac Shelter is located in a local hotel occupying 99 rooms.

 

POSITION DESCRIPTION:

The Case Manager is responsible for providing case management service to those individuals enrolled in the shelter program. The Case Manager will coordinate screening, intake and case management service at the locations. The Case Manager will work with clients during the day and will also engage with clients during shelter hours.

Responsibilities

MAJOR DUTIES AND RESPONSIBILITES:

  • Provide screening, intake, and case management for the SKC Shelters’ program participants.
  • Conduct triage assessments for the Coordinated Entry for All system in a trauma informed manner.
  • Meet with program participants to assist with completing paperwork and documentation for community supports such as SSI, TANF, ABD, Medicaid etc
  • Attend housing appointments with clients and housing providers to ensure client advocacy and comprehension. 
  • Work primarily on goals related to obtaining housing and/or increasing incomes.
  • Work with clients to develop plans for stability, including the areas of housing, job search, substance abuse and treatment, physical and mental health, personal development, and other life domains.
  • Establish connections to build rapport and empower clients.
  • Provide referrals and advocate for program participants with other social service agencies, hospitals, mental health agencies, and treatment centers.
  • Meet with program participants to assess housing options and basic needs.
  • Provide follow up supportive services to program participants working toward employment goals.
  • Crisis intervention and de-escalation as needed.
  • Complete assessments in order to identify potential King County Coordinated Entry system resources.
  • Connect clients to other CCS programs such as the two South King County Day Centers for mail, showers, warming centers, etc.

 

Provide case management for the individuals that enter the Federal Way Day Center and Kent Community Engagement Center.

  • Work with clients to develop individual goal plans. Work with individuals to monitor progress, provide advocacy-based information and referral, including life skills, budgeting, jobs/education, crisis invention and mental health.
  • Conduct triage assessments for the Coordinated Entry for All system
  • Ensure accuracy and maintenance of all client records, including service notes and outcomes; prepare reports as indicated by Supervisor.
  • Work with outside service providers to incorporate more services and collaboration.
  • Ensure that any Incident reports are written and given to the Supervisor for review.
  • Represent the interests of SKC Shelter Services program by attending meetings in the community.
  • Conduct outreach to sites where people who are experiencing homelessness frequent.
  • Proficiency in maintaining case notes and client information using HMIS and Shiftnote.
  • Daily logging and observation of shelter activity in Shiftnotes.
  • Regularly review SKC shelters case notes to ensure accuracy.
  • Creation and maintenance of client profiles in HMIS.
  • Maintain Clarity files and ensure all necessary documentation is included.
  • Provide quarterly summaries of client successes.
  • Ensure profiles are in compliance with agency and privacy standards.

 

JOB CONDITIONS:

This position requires the employee to work in an environment where there may be exposure to blood, bodily fluids, and other potentially infectious material.  Other exposures could include cleaning supplies, chemicals involved in pest control, paint and other materials used in building maintenance. This position is located in a hotel. Clients in this harm reduction location may have severe mental health concerns and/or substance abuse barriers.

  • Able to hear telephone rings, phone and face-to-face conversation, and emergency alarms.
  • Able to speak clearly in person and on the telephone.

  • Able to read normal size print and handwritten notes.
  • Able to sit for sustained periods of time.
  • Mobility/dexterity of hands/arms to enable keying into locked areas as well as using a computer and other office equipment.
  • Able to make independent decisions and apply sound judgment in performing job duties.
  • Regularly able to perform duties as assigned.

Qualifications

MINIMUM QUALIFICATIONS:

  • High School Diploma or GED equivalent.
  • Two (2) years direct experience with homeless and/or at-risk populations.
  • At least one 1 year of case management  or equivalent experience.
  • Knowledge of social services resources and programs in King County.
  • Ability to support shelter rules, including maintaining a safe and healthy environment.
  • Ability to work with culturally diverse clients.
  • Ability to work independently and with a team.
  • Computer skills and ability to learn and use CCS HMIS.
  • Commitment to CCS's Mission Statement.

 

PREFERRED QUALIFICATIONS:

  • Bilingual (English/Spanish)
  • Familiarity with King County resources
  • CPR/First Aid Certificates/Blood Borne Pathogens
  • Valid WA State Drivers License and auto insurance, and ability to meet CCS Driving Policy requirements.
