Dover Hospice, a regional healthcare provider and true friend of adults facing terminal illnesses, seeks a service-minded, career-oriented individual to join our team as a PRN Hospice Social Worker at our Glen Carbon and St. Louis locations.

As a Hospice Social Worker, you will impact the lives of our patients by
- Provide Medical Social Services to the patient and/or family according to the Plan of Care under the direction of a physician.
- Conducting a psychosocial assessment as part of the comprehensive assessment per agency policy. This includes emotional, social, financial, and environmental resources and to identify appropriate psychosocial problems, interventions, and goals for the patients plan of care.
- Participate in IDG meetings and the development of the patients plan of care
- Observe, record and report changes on the patients emotional and social factors that are being affected by the patients illness and his/her need for care and his/her response to treatment
- Maintain and submit written clinical records
- Evaluate the patients and familys response to and effectiveness of the medical social work intervention
- Participate in IDG and QAPI meetings and activities
- Participate in discharge planning, as needed.
- Assess patients/family members ability to cope with the patients dying
- Function as consultant to the members of the health team, assists them in understanding the social, emotional and environmental factors related to the patients health problems
- Help patients to utilize the resources to their families and the community
To become a Dover Hospice Social Worker, you will need
- Has a Master of Social degree from an accredited school of social work
- One year of social work experience preferred
- Hospice or home care experience preferred
- Must be a licensed driver with an insured automobile in good working order, and clear driving record. Current and valid drivers license, reliable transportation and current liability auto insurance is required.
- The ability to communicate well, both verbally and in writing
- Must be able to meet all physical demands of the job which includes, but is not limited to seeing, speaking, touching, walking, standing, bending, reaching overhead, crouching/kneeling or crawling, as well as the ability to regularly lift/move up to 50lbs independently and up to 200 lbs. with assistance.
- Ability to work well with others and take direction from management, as well as take initiative-willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
- Self-motivated to accomplish identified goals with a strong sense of accountability for results.
