Blended Case Manager

CenClear, a provider of mental health services, is seeking a dedicated and compassionate Blended Case Manager (BCM) to join our team in Blair County, PA. In this full-time role, you will collaborate closely with individuals and families to provide support, advocacy, and case management services for those suffering from serious mental illness or emotional disturbance. Responsibilities

• Provide case management services to adults and children with serious mental illness or emotional disturbance
• Work with individuals and families to access, link, coordinate, and monitor needed services
• Provide support and advocacy for clients and their families
• Maintain client records and documentation
• Collaborate with other professionals and agencies to ensure comprehensive care
• Adapt work hours to the needs of the position, including occasional evenings, weekends, and on-call emergency services
• Work 40 hours per week, 52 weeks per year

Qualifications

• Bachelor's Degree in a relevant field (e.g., sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education) or a registered nurse; or a high school diploma and 12 semester credit hours in a relevant field plus 2 years of direct contact experience with mental health individuals
• 1-3 years of experience working with children and/or adults in a mental health setting preferred
• 1-3 years of case management experience preferred
• Excellent verbal and written communication skills
• Valid driver's license and access to a reliable automobile
• Ability to work in various settings, including individuals' homes and community locations
• Strong organizational skills and ability to remain composed in pressured situations
