Location

Position is based at our MH/SUD outpatient facility located in Bellefonte, PA

Hours

This is a full-time position working 40 hours per week, 52 weeks per year

Qualifications

  • Bachelor’s Degree in mental health or related field
  • Knowledge of staffing processes
  • Knowledge of MH/SUD programs
  • Knowledge of community resources
  • Must be able to adapt hours to needs of position
  • Must possess excellent verbal and written communication skills
  • Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate

Responsibilities

  • To ensure consumers’ needs are met when a therapist has not yet been assigned or in a staff absence, that provided services are billed when appropriate.
  • Develop individualized, recovery-oriented emergency and medication only treatment plans.
  • Perform non-billable therapeutic tasks, such as talking to outpatient consumers on the phone/in-office regarding funding stream issues or referrals for services etc.
  • Coordinate care for consumers who are hospitalized or returning to services after a hospitalization or episode of residential care.
  • Utilize shared decision making in order to promote growth and change by participating in the development of procedures and policies for MH/SUD programs from a quality assurance perspective.
  • Monitor HIPAA compliance.
  • Aide the Records Room staff in the request for records when applicable
  • Openly communicate to the appropriate supervisory staff when a “Red Flag” issue is found in the review of any Mental Health Division paperwork.
  • Complete phone calls to consumers in identified timelines to assess satisfaction and obtain suggestions.
  • Openly communicate successes and suggestions found to Program Director/Regional Directors/Supervisors.
  • Communicate via phone or face-to face interaction with consumers who are not engaging in treatment to evaluate barriers in order to support consumers through their growth and change.
  • Oversee staffing process (should this be moved to the staffing section below)
  • Consult with Clinical Supervisors/Directors on appropriate staff assignments.
  • Monitor and provide assistance meeting non-treatment needs for each consumer from enrollment to discharge.
  • Develop and ensure maintenance of an accurate list of open consumers and their assigned therapist.
  • Assist consumers with referrals to other treatment services within or outside of the agency at time of discharge.
  • In the spirit of social responsibility, ensure EHR workflows are followed.
  • Ensure all intake documentation is complete and accurate.
  • Review recommendations for services to ensure all are implemented.
  • Facilitate referrals for additional services based on recommendations document completed work in Credible.
  • Assure follow-through on aftercare plans as needed following discharge.
  • Openly communicate with all team members to assure proper assignments are made when assigning cases.
  • Communicate to all parties involved in the coordination of services for consumer (start date confirms first date of services).
  • Ensure a list of community resources and funding sources are available to our consumers.
  • Review psychiatric and psychological reports for recommendations and ensure that all who need to know have information.
  • Consult with psychiatrist on follow-up items identified by the treatment team as applicable.
  • Notify the Regional Director, Program Director, and Senior Operations Officer of any critical/emergent situations that occur within your designated site/clinic.
  • Meet with Regional Director monthly.
  • Attend and actively participate in meetings in non-violent, emotionally intelligent ways with an openness to growth and change.
  • Complete all necessary documentation to funding streams regarding critical incidents. (CIR)
  • Provide periodic reports on outcomes based on key result areas as requested.
  • Follow-through on all procedures and directives pertinent to case management, consumer care and quality assurance, assuming responsibility and accountability for completion.
  • Complete tasks not listed above but which would be appropriate this position.
