POSITION DESCRIPTION
The full time Care Coordinator is responsible for arranging, coordinating, monitoring, evaluating, and advocating for links to appropriate services to address client needs. This includes meeting with a case load of up to 25 clients per month, addressing their needs, concerns, and psychiatric emergencies as needed. This is a full-time, exempt position, reporting to the Coordinator of Community Support Services and/or Case Manager Supervisor.
KEY JOB RESPONSIBILITIES
- Maintain an active case load of a maximum of 25 clients per month.
- Conduct meetings with clients to address identified needs, concerns, and psychiatric emergencies and, when necessary, facilitate referrals to outside agencies.
- Conduct intakes to evaluate client risk and assess needs as well as completing all initial documentation required by COMAR regulations for referred clients within appropriate time frames.
- Conduct comprehensive assessments and re-assessments to identify needs and areas of concern.
- Develop Plan of Care (POC) that addresses client’s case management and service needs by identifying goals and interventions.
- Arrange, coordinate, monitor, evaluate, and advocate for links to appropriate services to address client needs.
- Provide supportive counseling and advocacy for clients as needed and engage clients in the problem-solving process.
- Advocate on the behalf of the client and act as a liaison by maintaining contact with external agencies, and request supporting documentation from therapists, schools, doctors, or other agencies, as needed to support management of the case.
- Assist with the collection of outcome measures such as Family Satisfaction and Client Satisfaction Surveys.
- Represent the program to outside agencies, including the Core Service Agency.
EDUCATION AND WORK EXPERIENCE REQUIREMENTS
- BA/BS degree in human service field.
- Ability to handle stressful situations.
- Understanding of cultural diversity.
- Ability to observe and accurately report the functioning of children and families.
- Valid driver’s license and vehicle insurance
- Successfully complete State and Federal background investigation and drug test
PHYSICAL REQUIREMENTS/ENVIRONMENTAL CONDITIONS
Requires frequent travel to meetings and client homes and engagements.
JOB TYPE
Full Time; Exempt
Hours include daytime shifts and two evenings per week.
PAY
$37,000 - $40,000 per year, commensurate with experience
