THE JOB
As a Care Manager, you'll help individuals and families in our services to access local resources, medical care and medications, employment or financial resources, social support, and more. In order for program participants to maintain their housing, you will frequently stop in and check on them, be present around the housing locations often for assistance, and assist them in activities needed to maintain a clean living space. As an active member of the treatment team, you will work alongside medication prescribers, licensed therapists, crisis response teams, and various agencies and organizations throughout the community to help the participants obtain the services and resources they need. As each individual continues along their recovery journey, you'll provide support, crisis intervention strategies, and will work directly with each individual to adapt their treatment plan to fit their unique strengths, needs, and goals as they arise.

WHAT YOU HAVE
Required:
- High school diploma or equivalent
- Two or more years of experience working with youth and adolescents who have behavioral health needs
- Knowledge of co-occurring disorders and related needs
- At least 21 years of age
- Valid driver's license with acceptable DMV record (last 7 years)
Bonus Points:
- Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred
