Centerstone respects any decision that prospective applicants make regarding receiving a COVID-19 vaccine and will process requests for accommodations in accordance with applicable law.” Position Summary: This member of the multidisciplinary team has responsibility to carry out rehabilitation and support functions for persons with substance use and mental health disorders. This member assists in treatment under the clinical supervision of staff with professional degrees. If candidate is not a Certified Peer Specialist (CPS) upon hire, it is required they achieve their Certification within the first year of employment. Minimum Qualifications: High school diploma and minimum one year experience working with adults/families with mental health disorders and substance use disorders. May have case management related training with one year experience working with adults with substance use and mental health disorders. Valid Florida Driver's License with good driving record and valid auto insurance is required. Special Skills, Knowledge and Abilities: Knowledge and understanding of the characteristics and challenges that face clients with substance use and mental health disorders. Building connections with clients and their families as well as connecting them to community supports and ongoing following up on those connections. Willingness to work on a comprehensive treatment team where excellent communication, high motivation, self-direction, and willingness to do whatever it takes occur at all times. Flexibility to work a variety of hours, including evening and on-call to meet the needs of consumers. Essential Functions/Job Duties: • Create and build connections with clients to facilitate recovery and assist them in meeting their needs in order to become self-sufficient and maintain their recovery. • Develop therapeutic rapport with clients and demonstrate a recovery-oriented approach. • Provide case management and outreach services to clients while building program caseload. Assume responsibility for assessing client and family strengths, challenges, and needs. • Obtain collateral information and collaborate with clients, their families, support systems, outside providers and involved agencies in the community regarding progress in gaining self-sufficiency and promoting recovery. • Demonstrate knowledge of community resources to link individuals in accessing and learning about related community resources and ancillary services. • Referrals - Complete, document and follow up on referrals including collateral contacts, discharges, etc. via electronic health record note entry and team communication. • Other Related Duties – consultation and participation in treatment team meetings, collaboration with community partners, adherence to departmental and agency policies, procedures and processes, state reporting, etc. • Maintain or Obtain (within one year) Florida Certified Peer Specialist certification and adhere to all training, ethics and appropriate boundaries noted therein. Work Environment/Physical Demands: This position is located primarily in Manatee County. A required Travel to Manatee Memorial Hospital is required as part of the initiative to engage and support access to services. Certified Peer Specialists are required to work in the field in varied environments. If employed in hospitals/offices, work in clean, well-lit establishments, but they can be exposed to various health conditions. Sits, stands, bends, lifts and moves frequently during work hours. Is subject to frequent interruptions. Is involved with clients, family members, Centerstone staff, visitors, government agencies/personnel, etc. in a behavioral health, correctional and community settings. Position Type, Expected Hours of Work and Travel: This is a full time, 40 hour position. This position requires flexibility in work hours to include some evening and weekend hours as needed.
