Job Description: Do you enjoy working in the community and helping others? As a Behavioral Health Nurse/LPN with the Health Home Program you will be working to promote health-focused growth and change with client in the community setting. Behavioral Health Nurse LPN: Provides medical care coordination with internal programs and external agencies Collaboratively works with health home clinicians and peer wellness coaches Completes vitals and medication monitoring Provides health education Completes Health Risk Assessments Attends weekly clinical staffing and team meetings Participates in monthly development with supervisor Qualifications/Requirements: Practical nurse license under the Nurse Practice Act Valid driver’s license and safe driving record Must successfully complete all required background screenings.
