Job Description: Entry level starting salary: $16.50. /hr. As the first point of contact for families referred to the program, the FAW completes the initial screening and assessment to determine eligibility for services and arranges for services and supports. More specifically the FAW will conduct regular assessments of each family enrolled, documenting progress and consulting w/FSW re: need for further intervention, referral or assessment. Job Qualifications: Bachelor’s degree in psychology, social work, or other behavioral health care field. Essential Duties and Responsibilities: Provides ongoing and continuous outreach to build relationships with community referral sources. Serves as the main point of contact for incoming referrals for the designated area. Completes in home screening and assessments on all families referred to program. Maintains required documentation. Assist in preparation of required monthly, quarterly and annual statistical reports. Maintains up-to-date and accurate records for completed assessments and other required records. Participates in training according to requirements of HFA guidelines, GKI and DOH contract. Provides regular assessments as required for all enrolled families residing in the designated area. Attends training and other mandatory meetings Collects data required by funding source to document compliance. Performs other related duties as assigned Regular and Reliable Attendance is required.
